We are aware that a former employee has been charged criminally for an event that occurred at our facility. Ortonville Area Health Services and FWV continues to remain deeply saddened over the situation. Please note the individual who is being charged is no longer affiliated with or employed by our facility. Throughout the process following the event, Ortonville Area Health Services has fully cooperated with law enforcement, and we will continue to do so. We thank law enforcement for their dedication and work. Our hearts continue to be with the family, and our main focus is how to continue to support them. As an organization, we are committed to delivering compassionate, high-quality care, and the alleged actions of this individual are completely inconsistent with the standards of conduct and care we uphold. We hope that our long-standing record of service and dedication to the community provides reassurance that this incident does not reflect the values or level of care provided by Ortonville Area Health Services.

Fairway View Neighborhoods in Ortonville has been recognized in the recently released 2026 Best Nursing Homes ratings by U.S. News & World Report. This marks the second consecutive year that Fairway View has earned this achievement. Nearly 15,000 nursing homes nationwide were evaluated using 17 quality measures, with fewer than 19% earning the “Best Nursing Home” distinction. The analysis is based on data from the Centers for Medicare & Medicaid Services (CMS) and assesses performance in both short-term rehabilitation and long-term care.

Since its debut in 2009, the annual Best Nursing Homes report has served as a key resource for families, offering trusted, data-driven insights for those seeking either short-term rehabilitation or comprehensive long-term care.

A Neighborhood-Based Approach to Skilled Nursing

Fairway View Neighborhoods is a skilled nursing community with a focus on independence, dignity, and high-quality care for older adults. Its care model centers on three 17-bed neighborhoods, each featuring a kitchen and dining room. This home-style structure emphasizes familiarity, routine, and resident choice, attributes increasingly valued in today’s senior-care environment.

Virtual Reality at FWV: Exploring the World from Home

We’ve been having lots of fun testing out our new virtual reality (VR) equipment at Fairway View! This technology comes from Rendever, a company that specializes in VR technology for senior living communities. The system offers an expansive library of VR experiences and learning features.

When someone puts on a headset, they’re fully immersed in the experience. Meanwhile, everyone else can follow along on the large projector screen. This way, the entire group can share the adventure together.

Everyone has reported that the VR headsets are very comfortable to wear.

Above: The big screen shows what is being presented on the VR headsets, making this a great group activity. But, while those watching the big screen might just see an elephant, those wearing headsets are so immersed they might feel like they can reach out and touch the animal.

Don't forget to look up! The VR headsets provide a 360 view, meaning you can look up and see the sky above you! The photo above was taken during the Northern Lights experience.

The headsets are easy to take on and off and can be quickly disinfected between uses, ensuring everyone has the opportunity to enjoy them.

Dreams Come True with Help from Local Pilots and Fairway View

Ever since Gene Baldwin was a young boy growing up in Fergus Falls, Minnesota, he loved watching the crop dusters fly over his home. He was engrossed with how they would swoop down, charge back up into the sky, and angle off for the next round.

Gene currently resides at Fairway View Senior Communities in Ortonville, Minnesota. A few weeks ago, when the Neighborhood Leader at Fairway View asked Gene what he would like for his birthday, Gene replied, "I would like to have a ride in a crop duster, but I know that is just a dream."

Wondering if she could make that happen, the Neighborhood Leader called Milbank Airport and spoke with Christian Reimche of Reimche Airspray, who informed her that crop dusters are one-seaters and cannot fly a passenger. Christian then offered to help find a pilot for a plane ride and put her in contact with Dan Loehrer, Commercial Pilot and Certified Flight Instructor.

Visitation is OPEN!!! Fairway View Senior Communities is very (and we mean VERY) excited to announce that visitation restrictions have relaxed, and we are open for visitors again. This has been a long time coming, and we know residents are excited to see loved ones after a long 12 months. Following CMS and MDH guidelines, we have a plan to allow visitation to resume with residents safely. Indoor Visitations at FWV Assisted Living

We are offering visiting hours 7 days a week from 9am to 5pm. To do so, please go to https://calendly.com/fwvalmc/indoor-visitation

We ask that you do wear a mask when visiting your loved one.

There are a few different ways of communicating with your loved one in this visiting area. There is a pocket talker in the family area that you can talk into and the resident will wear headphones to hear you. There is also an Alexa Dot in with the resident. If you would like to use this, please have your resident say, “Alexa, dial ‘whatever your phone number is, including your area code” and you can talk thru that. Some people may be able to talk thru the door without needing any aides.

If your loved one needs assistance coming down to the visiting area, please call staff prior to you getting here so the staff have time to get them for you. We do ask that you limit visits to one hour so others can utilize this area as well.

We also ask that when you are done with your visit, please sanitize the inside door that you faced when visiting with your loved one prior to you leaving. The necessary items will be in the visiting area.

Indoor Visitation to resume on January 14th, 2021

Fairway View Neighborhoods is excited to announce that indoor visitations will be starting up again on Thursday January 14th. Because we have gone 14 days with no positive tests AND we have been below 10% in the county for 14 days, we are able to open to visitors again. If the county rate goes above 10% again, please know that visitation will be cancelled. Please read the following regarding visitation:

All Indoor Visitations must be scheduled through Calendly. Use this link to schedule any visits. https://calendly.com/fwvneighborhoods/visitation

All visits will be done in the visitation room in the Town Center. Visitors will be limited to 3 MAX during one visit.

Staff will maintain visual observation but provide as much distance as necessary to allow for privacy of the visit conversation.

Visitors will be screened for signs and symptoms of COVID-19 at the Town Center entrance.

Face masks MUST be worn at all times

Social Distancing MUST occur during the visit

Access to the inside of the building or bathrooms is limited. Please plan ahead.

PHYSICAL CONTACT WILL NOT BE ALLOWED. No hugging/kissing, touching, or physical contact will be allowed.

We are very excited to share the news that we will soon be able to resume indoor visits, and we warmly welcome you back. Minnesota Department of Health (MDH) issued guidance on how we can safety re-open to visitors and continue to protect the health of our residents and staff. We are currently developing a safe and comprehensive plan and expect visits to resume on 10-19-2020.

Our ability to resume visitation would not have been possible without the incredible commitment and dedication of our staff, who have worked tirelessly to manage the risks of COVID-19. We know that you, too, have made sacrifices during this incredibly difficult time and we thank you for your patience and understanding.

The guidance provided by MDH is quite detailed, and we appreciate your patience as we take time to evaluate and implement the criteria so that we can safely resume visits. Because we have zero cases of COVID in the Neighborhoods we are able to resume visits. If our facility experiences any positive COVID cases with staff or residents OR our county transmission rate is above 10%, visits will be suspended until it is safe to start again. If you have been assigned as an Essential Caregiver, this letter doesn’t pertain to your situation and you will be allowed to continue your caregiving duties as normal.

The following is a list of things you can expect if you would like to visit a family member or friend:

· Your visit must be scheduled per the guidance. This allows us to monitor the number of visitors in our building at any given time. Please see attached page for instructions.

· Expect to be screened for COVID symptoms upon entrance to the building.

· Visitors will need to wear a mask or other face covering at all times.

· Visitors will need to sign in and out and leave contact information.

· Stay at least 6 feet from others (social distancing)

· For all visits, please enter through the Town Center door, where you will be escorted to a visitation area.

· Please see the attached page for Frequently Asked Questions and further instructions.

If the above guidelines are not able to be followed, the visitor will be asked to conduct their visit through another option such as a virtual visit or a window visit.

Thank you again for being partners with us in fighting the COVID-19 virus. Your messages of support, prayers, and caring will not be forgotten. How do I schedule a visit?
All visits will be scheduled through an online scheduling system. To schedule a visit:

· Go to https://calendly.com/fwvneighborhoods/visitation

· A calendar will appear. Pick your intended date of visit. Once you click on the date, a list of open time slots will appear.

· Click on the requested time slot you would like

· Enter your contact information and click SCHEDULE EVENT when finished.

Please do not come for a visit if you feel ill, are demonstrating symptoms related to COVID-19, have been in contact with someone who is COVID-19 positive in past 14 days or have tested positive for COVID-19.

Will there be scheduled hours for the visitation?

Yes. When you schedule your visit online you will see that there are a limited number of visitations per day. Each visit can be up to one hour in length with a max of 3 visitors at one time per resident.

Will there ever be a circumstance where a visit is denied?
It is our goal and intention to meet all requests for visitation. However, we retain the right to deny visits based on the status of COVID-19 in the building or the risk of virus transmission in our community. For example: if a resident or staff is diagnosed with COVID-19, visitation will be restricted until our setting has been COVID-19 free for 14 days. Consistent with state and federal guidance, we also regularly monitor the level of COVID-19 transmission in our broader community, and changes in the transmission rate may also lead to changes in visitation policy.

A visit may also be denied if a visitor shows signs or symptoms of COVID or has a known exposure to COVID. Further, failure to follow the guidance from MDH could result in the denial of a visit.

What can I expect when I arrive for my visit?

You will be screened for COVID-19 symptoms upon arrival, and asked to use alcohol-rub sanitizer. You will be escorted to the visitation area where the visit will take place.

Am I allowed to give my loved one a hug or hold their hand?

No. Due to the increased risk of exposure, holding hands, hugging, kissing, and other physical contact is not allowed during visits. A wave is the safest way to greet the resident.

Is the visitation open for all ages?

Yes. Please know all visitors will be asked to comply with our visitation protocols regardless of their age.

Are there any other type of visitation allowed now?

Yes, outdoor visits, window visits and visits through alternative modes of communication (such as Zoom, Skype or FaceTime) are also available. We also continue to support compassionate care visits and designated individuals as essential caregivers when appropriate.

What if I am wrongly denied visitation with my loved one?

If you believe you have been wrongly denied visitation, or you have questions that aren’t answered by these FAQs or the guidance, you may contact the Ombudsman for Long-Term Care at 651-431-2555 or 1-800-657-3591.

FWV Catered Living Tenants & Assisted Living / Memory Care Residents Test Positive for COVID-19

Recently, 3 residents of our FWV AL/MC have tested positive for COVID-19. Additionally, over the past two weeks, two Tenants of our FWV Catered Independent Living have also informed us that they have tested positive for COVID-19 and are isolating. We are keeping our Residents, Tenants and their primary contacts informed with routine updates via phone calls, letters, texts, and weekly newsletters.

Please continue to support our FWV Family by supporting our staff and reducing the spread of COVID in the area.

All Fairway View Staff tests negative for COVID-19

On Monday, September 14th, all of our 160 FWV staff and contracted staff who serve our residents at Fairway View Senior Communities completed our first round of COVID-testing as required by Medicare. We are pleased to share that all 160 staff members tested NEGATIVE for COVID-19. This is wonderful news! We are so appreciative of all that our staff gives of themselves to serve and protect our residents. Thank you to everyone in our organization and community who continues to take the necessary precautions to limit the spread of COVID and protect our seniors and those most vulnerable.

The next round of weekly testing, as required by Medicare, will take place on Monday. Per Medicare, effective September 2nd, all 15,600 nursing homes, serving 1.5 million residents across the United States, are required to test all staff for COVID-19 at least one time per month to at most twice per week based on the positivity rate in our county.

Fairway View Neighborhoods Update- 4/30/2020

We are reaching out today to keep you updated on the current situation at the Neighborhoods. As you know, last week we had a resident test positive for coronavirus (COVID-19). We have determined that this resident had no direct contact with any other residents in the Neighborhoods. At this time we have no other suspected or confirmed cases with residents or staff. We can assure you that your loved one is healthy and doing well. We continue to prioritize the safety and well-being of our residents and employees, and are actively preventing the spread of this illness.

We are working with Minnesota Department of Health to respond to the situation. We are following their guidance in preventing further spread of this infection.
Our staff have moved from wearing cloth masks to medical grade masks. When they leave work at the end of their shift, we are asking staff to wear cloth masks for their essential outings in the community to further protect themselves from bringing in anything further to the residents.
We continue to follow CDC infection control protocols.
We continue to screen our employees at the beginning and end of their shift for respiratory symptoms including fever and cough.
